We will perform our Certified Nursing Evaluation and provide you with specific information about your client’s current status as well as immediate and anticipated needs, going forward. We’ll also provide you with a professional Customized Care Plan (CCP) to address all areas of concern.

Our care managers are an excellent resource for identifying appropriate assisted living facilities or skilled nursing facilities, as well as assisting with the emotional transition and physical move to the next level of care.

Our care managers maintain close contact with designated family members (or other approved-contacts) to keep them up-to-date with their loved one’s condition. We keep in touch with families through telephone calls, email, text messaging, postal mail and fax.

Our care managers provide fully detailed, comprehensive reports which include all assessment areas (physical, medical, cognitive, functional, fall risk, home safety and social) and include specific recommendations to meet all identified needs.
