What is a Plan of Care?

A Plan of Care is created based on the
unique medical and emotional needs
identified at the Initial Assessment.
A Plan of Care may recommend some
of the following services:

  • Guidance and assistance with identifying
    and obtaining available government programs
  • Personal visits in the hospital setting
    to ensure quality care and appropriate
    discharge plans
  • Arrangement of home health care,
    companionship services or adult day care
  • Coordination of medical providers,
    appointments, and records, as well as participation at key medical appointments
  • Referrals to physicians, elder law
    attorneys, and financial planners
  • Medication set-up and monitoring of
    compliance with prescribed medication
    regime
  • Identification of community resources
    such as Meals on Wheels, senior groups,
    transportation, etc.
  • Arrangements for safety equipment,
    ordering, delivery, set-up and/or
    installation
  • Overall financial management, including
    coordination of monthly bill payments
  • Counseling about and coordination of
    relocation
     to a new living environment
    (assisted living, skilled nursing facility, etc.)
  • Errand assistance such as shopping for
    food and clothing, etc.
  • Regular, personal visits with your loved
    one to ensure that the Plan of Care
    remains appropriate to meet ongoing needs
