to ensure quality care and appropriate
discharge plans

appointments, and records, as well as participation at key medical appointments
- Referrals to physicians, elder law
attorneys, and financial planners
- Medication set-up and monitoring of
compliance with prescribed medication
regime
- Identification of community resources
such as Meals on Wheels, senior groups, transportation, etc.
- Arrangements for safety equipment,
ordering, delivery, set-up and/or installation
- Overall financial management, including
coordination of monthly bill payments
- Counseling about and coordination of
relocation to a new living environment
(assisted living, skilled nursing facility, etc.)

food and clothing, etc.
