What does a Care Manager do?

A Care Manager is a licensed professional
who assists you in ensuring that your
loved one is able to enjoy the most
independent, safe and dignified lifestyle
possible, while keeping you informed and
involved in decision-making along the way.

A Care Manager meets with clients
personally to perform an initial in-home
assessment of needs and to develop a
recommended Plan of Care.

An Initial Assessment includes
the following:

  • Review of all medical issues to
    ensure that appropriate medical care
    is being provided.
  • A safety assessment of the present
    living environment in consideration of
    physical limitations.
  • Social, psychological, cognitive,
    legal and financial assessments
     to identify all possible areas of concern.
  • A written report of the assessment
    and Plan of Care recommendations.
